Nurse Care Coordinator
Position: The Nurse Care Coordinator functions as a member of an interdisciplinary team to provide care to clients receiving Integrated Health Home (IHH) services. The Nurse Care Coordinator is responsible for assisting clients with medication adherence, appointments, referral scheduling, tracking follow-up results from referrals, understanding health insurance coverage, transition of care, wellness education, and health support or lifestyle modification.
Position Summary: The Nurse Care Coordinator provides support to clients experiencing Serious Mental Illness (SMI), Severe Emotional Disorder (SED), complex trauma and/or multiple comorbid medical or substance abuse disorders. The Nurse Care Coordinator advocates for and supports clients and their families, engages with community agencies/healthcare providers and others on their behalf to ensure access to needed services to increase wellness self-management and reduce avoidable emergency room visits and/or hospitalizations.
Education: The Nurse Care Coordinator must be a Registered Nurse (RN) or a Bachelor of Science in Nursing (BSN) with an active Iowa license.
Experience/Knowledge: The individual must have the ability to effectively communicate with clients, families, hospitals, and other providers in the community. Must have a professional and caring attitude. Some experience in integrated health home services is preferred, but not required.
Service Location: Polk County
Hours: Monday – Friday 40 hours per week
