HABILITATION SERVICES

Habilitation Services consists of individualized services and supports that assist with acquisition, retention, or improvement in skills related to living in the community. Habilitation Service activities and environments are designed to foster the acquisition of skills, appropriate behavior, greater independence, and personal choice.In order to participate in Habilitation Services a person must be involved with an Integrated Health Home.
