Position: IHH Care Coordinator provides support and services to individuals enrolled in our Integrated Health Home (IHH) program. The Care Coordinator manages client caseloads, conducting assessments and treatment plans, and facilitating conversations between clients’ providers.  

 

Position Summary: The Care Coordinator responds to the needs of clients with mental health disorders and needs. The Care Coordinator serves as a main point of contact in coordinating between providers and supporting the client. The Care Coordinator monitors and evaluates clients' progress.  

 

Education: The Care Coordinator must have a bachelor’s degree in a human services field. 

 

Experience/Knowledge: The individual must be able to effectively communicate with clients, families, medical providers, and other community providers. The Care Coordinator must have a professional and caring attitude and great communication and interpersonal skills. Some experience in integrated health home services is preferred, but not required.  

 

Service Location: Polk, Story, Warren, Dallas counties 

Hours: Monday – Friday (40 hours per week), some travel is required
