INTEGRATED HEALTH HOME

What is an Integrated Health Home?  

​

Our Integrated Health Home (IHH) program is designed to assist adults and children who have mental health disorders with support and guidance towards improving their overall health. It is a program that focuses on an individual’s whole-health and needs. Individual needs may include goals related to physical health, mental health, emotional health, spiritual, social, and community related goals. The IHH team works with you and your healthcare providers to coordinate your care, achieve your health goals, and most importantly improve your wellness.

While participating in our IHH program, you can expect the following:  

​

  1. Your Care Coordinator and/or other members of the IHH team will have contact with you to discuss your wellness goals and what we can do to help you achieve your goals 

  2. We will focus on and assist with a variety of your goals, including physical health, mental health, emotional health, spiritual, social, and community related goals  

  3. Your Care Coordinator will have regular collaboration and communication with your healthcare providers 

  4. We will build trusting relationships with your healthcare providers  

  5. As a team, we will work together to bring your healthcare providers and your individual goals together to achieve the best health outcomes possible  

  6. We will incorporate your input, prioritize your goals, and discuss and review your progress with you regularly

Who is eligible for IHH services?  

​

If Adult:  

​

  1. Must have Medicaid

  2. Must have a diagnosis of a Serious Mental Illness (SMI): Psychotic disorders, schizophrenia, schizoaffective disorder, major depression, bipolar disorder, delusional disorder, obsessive compulsive disorder or another mental health diagnosis with significant functional impairment  

  3. You live in one of the following counties that Stepping Stone Family Services serves: Polk, Dallas, Warren, Story   

 

 

If Child:  

​

  1. Must have Medicaid  

  2. Must have a diagnosis of a Serious Emotional Disturbance (SED): A diagnosable mental, behavioral or emotional disorder of sufficient duration to meet diagnostic criteria specified within the most current Diagnostic and Statistical Manual of Mental Disorders that results in a functional impairment. 

  3. You live in one of the following counties that Stepping Stone Family Services serves: Polk, Dallas, Warren, Story   

 

Note: IHH will manage the Children's Mental Health Waiver to assist families with accessing and utilizing services through the CMHW.
